TERMS OF ENGAGEMENT BETWEEN SARAH CRANAGE NUTRITION (REGISTERED NUTRITIONAL THERAPY PRACTITIONER - RNTP), AND THE CLIENT

“We”, “us”, “Sarah Cranage Nutrition”, “SCN” – refer to the clinic run by Sarah Cranage Nutrition
“You”, “client” – refer to any client of Sarah Cranage Nutrition
PART 1
The Nutritional Therapy Descriptor
Nutritional therapy is the application of nutrition and lifestyle medicine sciences in the promotion of health, peak performance and individual care. Registered Nutritional Therapy Practitioners assess and identify potential nutritional imbalances and understand how these may contribute to an individual’s symptoms and health concerns. This approach allows them to work with individuals to address nutritional imbalance and help support the body towards maintaining health.
Nutritional therapy is recognised as a complementary medicine and is relevant for individuals with chronic conditions, as well as those looking for support to enhance their health and wellbeing. Practitioners consider each individual to be unique and recommend personalised nutrition and lifestyle programmes rather than a 'one size fits all' approach.
Practitioners never recommend nutritional therapy as a replacement for medical advice and always refer any client with 'red flag' signs or symptoms to their medical professional. They frequently work alongside medical professionals and will communicate with other healthcare professionals involved in the client's care to explain any nutritional therapy programme that has been provided.
The Registered Nutritional Therapy Practitioner (RNTP) requests that the Client notes the following:
· The degree of benefit obtainable from nutritional therapy may vary between clients with similar health problems and following a similar nutritional therapy programme
· Nutrition and lifestyle recommendations will be tailored to support health conditions and/or health concerns identified and agreed between both parties
· RNTPs are not permitted to diagnose, or claim to treat, medical conditions
· RNTPs’ recommendations are not a substitute for professional medical advice and/or treatment
· Your RNTP may recommend food supplements and/or functional testing as part of your nutritional therapy programme and may receive a commission on these products or services
· Standards of professional practice in nutritional therapy are governed by the Complementary and Natural Healthcare Council (CNHC) Code of Conduct, Ethics and Performance If you are dissatisfied, please speak to SCN in the first instance. Any complaint should be submitted in writing to the CNHC for review.
This document only covers the practice of nutritional therapy within this consultation, and your practitioner will make it clear if he or she intends to provide services that are outside the scope of nutritional therapy practice
The Client understands and agrees to the following:
· You are responsible for contacting your GP about any health concerns
· If you are receiving treatment from your GP, or any other medical provider, you should tell him/her about any nutritional recommendations provided by SCN. This is necessary because of any possible reaction between medication and the nutritional programme
· It is important that you tell SCN about any medical diagnosis, medication, herbal medicine or food supplements you are taking as this may affect the nutritional programme
· If you are unclear about the agreed nutritional therapy programme / food supplement doses / time period, you should contact SCN promptly for clarification
· You understand that the recommendations are personal to you and may not be appropriate for others
· You must contact SCN should you wish to continue any specified supplement programme for longer than the originally agreed period, to avoid any potential adverse reactions
· Recording consultations using any form of electronic media is not allowed without the written permission of both yourself and SCN
· It is your responsibility to ensure you have a working internet connection for online appointments. If you are unable to connect as scheduled, the appointment may be conducted by telephone or failing that it will be considered a ‘no show’ and charged in full
· By becoming or remaining a client of SCN, you agree to these terms and conditions. These may be updated from time to time without prior notice. When updated, SCN will provide you with the revised version via the Penoch.io portal. It is my responsibility to read and understand the terms and conditions.
Confidentiality and Data Protection
The RNTP will keep your personal information confidential and secure following the UK GDPR guidelines for the practice. Records may be stored electronically. The RNTP will not share your information with third parties without your consent. However, if the RNTP believes there is a risk of significant harm to yourself or another person, the RNTP may pass the information onto an appropriate authority using the legal basis of vital interest. A separate Privacy and Consent Notice is available for your review.
Contact details may be used to send supplements, newsletters, or offers unless you opt out. Appointment reminders, reports, invoices, and receipts will be sent electronically.
PART 2
Online Patient Portal (Penoch.io)
The online portal allows secure sharing of evaluation forms, diaries, assessment results, and consultation reports. You can use it to:
· Complete pre-consultation questionnaires
· Communicate with practitioners
· Book appointments
· Access consultation notes, recommendations, and lab results
· Upload reports you wish to share
Payment Terms and Cancellation Policy
Fees are as stated on the SCN website, agreed in advance of the appointment and paid at the end of a face-to-face appointment or in full in advance of an online appointment. Payments can be made by bank transfer, credit or debit card. Payments are non-refundable unless mitigating circumstances apply (e.g., unforeseen ill health or personal crisis). In such cases, fees may be credited towards future consultations.
In the case of advance payments for services, you have the right to cancel this Agreement within 14 days of the purchase without penalty (‘Cooling-off Period’). If you wish to terminate the agreement within the 14-day Cooling-off Period, you must do so in writing to [email protected]. SCN will refund any monies paid prior to cancelling this Agreement. If the services have commenced during the 14-day Cooling-off Period, SCN will refund any monies due to you for services not yet provided. The refund payment will be made within 14 days of cancellation.
Appointments
Appointments may be booked via Penoch.io or by email. You will receive an email confirmation and reminder before your appointment. Appointments are usually held via Zoom. Please connect a few minutes before the scheduled start. If you have technical issues, message SCN via the Penoch.io portal.
In-person consultations may be available upon request. Please arrive 5 minutes early. If more than 10 minutes late, SCN may reschedule and charge up to the full fee. If your consultation is shortened due to lateness, the full fee still applies.
SCN may refuse future bookings if two or more appointments are missed without notice. Appointments cancelled with less than 48 hours’ notice and ‘no shows’ will be charged in full. Zoom appointments may be rescheduled subject to availability. Full cancellation with less than 24 hours’ notice is non-refundable. In case of unforeseen staff illness, we may need to reschedule at short notice.
Laboratory Tests / Supplements
All lab tests are commissioned from approved UK, EU, and US laboratories following practitioner advice. Test fees cover sample processing only. Interpretation and treatment planning require a separate consultation.
Your consultation plan may include supplement recommendations; direct ordering links will be provided.
